The Welsh Rugby Union is conducting an equal opportunities survey as part of its effort to attain the Framework for Sport Equality Standard at Foundation Level by the end of May 2011.
The survey will allow the WRU to assess the present level of representation of different groups within Welsh rugby, and all officials, players and club members are encouraged to complete the survey by Friday 4 March 2011.

It should take no more than a few minutes. Information will be treated as confidential in accordance with the Data Protection Act 1998. You will be given the option not to disclose information if preferred.
The club with the most replies by 4 March will receive a Wales shirt signed by the senior squad for display.
